Affordability Review
If you have had a change in your circumstances and would like to request a review of your case, it's important that you inform us as soon as possible.
This easy-to-use affordability review form will speed up getting the information to us.
Following your communication with our Customer Service team, we will have requested documentary evidence of your change in circumstances. Please complete the form below and attach your documents.
Your documentary evidence may include the following:
Proof of your current income e.g. your last 3 wage slips.
Proof of your unemployment e.g. P45/Letter from employer.
Proof of your redundancy e.g. Letter from employer.
Proof of your maternity leave & pay.
Proof of increased expenditure e.g. Invoice/receipt.
Statement of Fitness for Work (Provided by your GP).
Bank statements covering the last 3 months.
Any other documents that we have requested.
We have made this process easier than ever to do, simply follow the steps below to upload your documents and we will review them and get back to you.
You are obligated under the terms of your Trust Deed (Section 5 – Co-operation and Provision of Information) to inform your Trustee of any of the following:
- Change of employment
- Change of address
- New contact information (telephone number & email address)
- Change of income (e.g. salary, bonus, overtime, commission)
- Any other matters relating to your financial affairs
