Following your communication with our Customer Service team, we will have requested documentary evidence of your change in circumstances. Please complete the form below and attach your documents.
Your documentary evidence may include the following:
Proof of your current income e.g. your last 3 wage slips.
Proof of your unemployment e.g. P45/Letter from employer.
Proof of your redundancy e.g. Letter from employer.
Proof of your maternity leave & pay.
Proof of increased expenditure e.g. Invoice/receipt.
Statement of Fitness for Work (Provided by your GP).
Bank statements covering the last 3 months.
Any other documents that we have requested.

You are obligated under the terms of your Trust Deed (Section 5 – Co-operation and Provision of Information) to inform your Trustee of any of the following:
- Change of employment
- Change of address
- New contact information (telephone number & email address)
- Change of income (e.g. salary, bonus, overtime, commission)
- Any other matters relating to your financial affairs
